Membership Application
Dear Applicant,
Thank you for your interest in becoming a member of the Ninth Floor Club + Venue.
Authorized Agent and Designated Signers Instructions
As part of the business membership application, please complete the sections for both the
Authorized Agent and the Designated Signers.
- The Authorized Agent is the individual who is authorized to complete this application and act on behalf of the business.
- The First Designated Signer is required and will be the primary associate on the business membership.
- The Second and Third Designated Signers are optional and may be completed if your business would like to designate additional associates.
Cloud 9 Access
Your business may also elect to add Cloud 9 — The Ninth Floor’s private cigar bar & lounge for any associate who wishes to opt in.
Cloud 9 access is an additional $25 per month per associate, and a Yes/No option will be provided for each signer.
Cloud 9 access may also be added in the future if preferred.
Note: For business and nonprofit memberships, the Cloud 9 access fee is billed to the
business account as a single combined charge.
The business is responsible for allocating or recovering the cost from individual associates as needed.
We do not bill each associate separately.
Important:
If the Authorized Agent and the First Designated Signer are the same person, you must still complete both sections separately.
Each section must be filled out in full, even if the information is the same.
Please Note:
The membership welcome email and onboarding information will be sent to the Authorized Agent’s email address.
This will include important details such as how to set up Auto Pay for membership dues through ACH or credit card.
Lastly:
We encourage all signers to add their cell phone numbers for text confirmation related to club events.
